How Does A Gender Affirming Mastectomy Work?

Female-to-male top surgery or gender-affirming mastectomy is a very personalized procedure. Depending on the size of your chest and your contouring goals, your LPS physician may suggest either nipple-sparing subcutaneous mastectomy (for smaller chest sizes with minimal excess skin) or a subcutaneous mastectomy with free nipple grafting (for those with larger chests and bodies). Both procedures remove breast tissue along with excess skin for flatter chest contours. The nipple may be removed, resized, or repositioned depending on your aesthetic goals.

If you plan to take testosterone at any point, we recommend consulting with your primary care physician, psychiatrist, or endocrinologist before considering FTM top surgery to achieve an optimal outcome. Hormone therapy is typically not a requirement for subcutaneous mastectomy.

Am I a Good Candidate for Gender Affirming Mastectomy?

Female-to-male top surgery can have a positive impact on a person’s physical and mental health. If a flat, masculine, or male appearance is your desired outcome, you may be able to benefit from chest contouring. As with any surgical procedure, you should be a non-smoking adult who is in otherwise good general health. You should have realistic expectations about what top surgery can do for you.

Massachusetts is generally considered more advanced in what’s covered by insurance when it comes to gender affirming care and gender reassignment surgery. Check with your insurance provider to determine your eligibility for FTM top surgery coverage.

Visible results can be seen immediately after a gender-affirming mastectomy. The chest will have a more masculine and contoured appearance. As any post-operative swelling subsides, you’ll see continued improvement. Scars will start to lighten and fade in several months to a year. Most people who undergo the procedure are happy with the long-term results.

Recovery after top surgery varies from person to person. Patients can typically shower within 48 hours, and we will provide detailed aftercare instructions. You can generally expect to take about two weeks before returning to work or school after FTM top surgery.  More strenuous activities should be eased into your routine as you feel comfortable. It’s important not to push yourself too hard. Otherwise, you may risk prolonging the healing process.
